Care Plan setup - Step by Step Guide
1. Sign up on app.carecorner.net.au

We strongly recommend completing this process on a desktop / laptop or maybe a tablet. This cannot be done via the mobile app.

This process is directed via Auth0 - an authenticator system that helps us keep your data secure.

You will have to verify your email address via the link that is sent to your email address.

Then you can login and setup your care plan.
2. Patient Setup, Care Plan and Team Members
-
Enter the details for your Patient on the patient card.
-
Each account can only have one patient.
-
If you make a mistake, you can always come back and amend the info.
3. General Observations
-
Setup the Care Plan by starting with the General Observations you want to track.

You can track:
-
Temperature
-
Pain
-
Height & Weight
-
Mood
-

If you do not select any, neither of them will show on the mobile app - that is fine too.

If circumstances change, you can always select or deselect.
4. Daily Medications add, edit and delete

Medication charts can be setup in the Activities of Daily Living.

Start with your daily medications, you can add, edit or delete as things change.
-
You can determine the times that the medications are due and you can select various Routes of Administration.

Medication times will be populated on your mobile app with the relevant doses and a medication chart will be available in the Nursing Care Plan.

If you need to amend doses in the future. Delete the original dose and add a new one at the relevant time.
5. Weekly and other regularity medications

Continue with medications that are administered weekly or have other regularities.

Some of our users have medications that are only due every 3 days and others are taken every 2 weeks.

We allow you to customise your medication chart to fit your exact needs.
6. PRN medications and notes

Rescue Medications / PRN Medications have their own dedicated space.

Enter information as dosage, repeat times as well as the Indication to use the PRN medication.

This information will automatically populate a form on your mobile app and will display in the Care Plan as well.
7. (other) Activities of Daily Living

For other Activities of Daily Living the user can customise various forms and add information.

Nutrition: Add recipes, outline a bolus or continuous feed regimen.
-
Toileting: customise how much information you want to track and which means of continence management are in use.

ADLs: Select from a list of regular daily activities and add you own so they appear on the tracker

Sleep: add any information relevant to sleep
8. Neurology
The Neurology section allows the user to customise the forms for recording seizures and involuntary movements.

Select seizure and movement types to create a customised dropdown on the mobile app.

Specify timelimits for PRN medications.
​

Select triggers for seizures to customise the form and tailor it to your own needs.
9. Gastroenterology
The Gastroenterology section allows the user to add information relevant to Blood Sugar Levels and Ketones readings.
If BSL and Ketones need to be checked regularly, specify the times and they will appear on the Daily Activities list on the mobile App home screen.
10. Respiratory
In the Respiratory section the user can customise mobile app entry forms for the following areas.
Vitals: add times when you want vitals checked routinely.
O2 Therapy: Select the forms of O2 Therapy that are in use. Select the minimum O2 Saturation and add any additional notes.
Suctioning: Select if you want to track suctioning and add any information that should display in the Care Plan.
Chest Physio: Select if you want to track Chest Physiotherapy and add any information that should display in the Care Plan.
11. Allied Health

In the Allied Health section the user can customise the mobile app dropdown for Equipment.

Either select from one of the pieces of Equipment we have listed or create your own.

Add any notes relating to the equipment use as this will then appear in the Nursing Care Plan.
12. Emergency contacts and plans
Add as many Emergency contacts as you deem necessary. Specify their name, relationship and phone number. Mobile app users can access this data and directly dial the number.
We have populated three standard Emergency Plans (Seizures, Respiratory Distress and Pain). You can add custom Emergency plans and title them as you see fit. They all appear on the mobile app.
For each Emergency plan, the Account holder specifies:

Signs that indicate an emergency

Actions to be taken

Actions if signs persist

When to call the Emergency contact
13. Other information
In the Other information section the user can add free text for various categories. This information is automatically displayed in the Nursing Care Plan on the mobile app.
Categories include:

Important People

Things that comfort

Things that help is required for

Medical Equipment in use

Goals
14. Contact information
In the Contact information section the user can add Contact information for various medical or allied Health specialists. You can add phone numbers or email address which will appear in the Care Plan.
Default specialists include:

Physiotherapy

15. Inviting Team members
In order to give access to your team on the mobile app, you need to invite them to the Care Team. This can be done from the Dashboard, the Care Plan setup or the icon on the side menu directing you to the team.
Once you have everyone invited, you are set to go.
Your team members will receive an email, prompting them to download the CareCorner App. They sign up on the mobile App and will see your Care Plan.
